Help is just a phone call away. Care Navigator, For You is a benefit that connects you to a care navigator, a dedicated care expert who will help you find the right doctors and get you the services you need to feel your best.

Care Navigator, For You+ is available in some SCAN plans. The “+” indicates that you have this enhanced benefit and may be able to receive a comprehensive geriatric assessment. This evaluation is designed to improve your health and wellness and focuses on your unique physical, mental, and social well-being.

Ask a care navigator if you have this enhanced benefit and if it would be of value for you. Your care navigator will also help you understand the results of the assessment, any recommendations, and create a personalized care plan to support your health and independence.

You’ll work directly with a dedicated care expert to answer your plan and healthcare questions. They’ll guide you on how to use your benefits, find doctors, and help schedule your appointments.

How do I get started with this benefit?

First, confirm that your SCAN plan includes Care Navigator, For You by checking your Evidence of Coverage.

If so, call SCAN Member Services at (800) 559-3500 (TTY: 711) to connect with a care navigator. They’re available from 8:00 a.m. to 8:00 p.m. seven days a week from October 1 to March 31, and Monday through Friday from April 1 to September 30.

When should I reach out to my care navigator?

Reach out to your care navigator to:

  • Help you understand your plan and how you can get the most from your benefits.
  • Find and schedule appointments with doctors or specialists.
  • Coordinate transportation to and from doctor’s appointments and care visits.
  • Set up preventive care like colon cancer and blood pressure screenings, as well as arrange your annual flu shot and other needed vaccines.
  • Coordinate care between different healthcare providers so you don’t have to worry about arranging referrals and follow-up care.

Do I have to pay to use the Care Navigator, For You benefit?

The care navigation benefit is available at no cost to members with a SCAN plan that includes this benefit. To find out if your SCAN plan includes Care Navigator, For You, check your Evidence of Coverage or call SCAN Member Services to see if this benefit is available to you:
