SCAN offers a network of primary care doctors, specialists, hospitals, and other healthcare providers. You must receive all routine care from in-network plan providers except for emergent or urgent care situations or for out-of-area renal dialysis. If you obtain routine care from out-of-network providers neither Medicare nor SCAN will be responsible for the costs (Exception: VillageHealth (HMO-POS SNP) plans. For more information, please contact the plan. Or refer to the plan’s Evidence of Coverage (EOC) for more details). The Formulary, pharmacy network, and/or provider network may change at any time. You will receive notice when necessary.
