    This is sent every September. It outlines any changes to your SCAN plan for the coming year.

Personalized Notifications

  • Health Check Record
    This personalized record of your care is mailed once a year; the online version is interactive, updated monthly and available 24/7
  • Personalized health information
    Includes personal health information, such as specific medication names or other private info
  • Portal notifications
    We currently email you when you have a message in your SCAN member account. Uncheck the box if you don’t want these emails
