What are SCAN’s Chronic Condition Special Needs Plans (C-SNPs)?

If you have diabetes, end-stage renal disease (ESRD), lung conditions and/or heart disease, SCAN has a Chronic Condition Special Needs Plan customized to meet your unique needs. SCAN C-SNP plans are Medicare Advantage plans with benefits to support people with one or more of these chronic conditions. Our Chronic Special Needs Plans (C-SNP) offer tailored benefits and deliver comprehensive care, helping to improve the health of our members with chronic conditions.

With SCAN’s C-SNP plans, you’re covered for doctors’ visits, treatments and resources to help you stay healthy and independent. You’ll also have many extra benefits not covered by Original Medicare, including:

Depending on where you live, you may be eligible for a SCAN C-SNP plan if you have one or more of the following conditions:

  • Diabetes
  • End-stage renal disease (ESRD)
  • Congestive heart failure
