The ISNP Embrace Plan is a Medicare Advantage plan specific for residents of long term care communities. It is designed to help keep you as healthy as possible without having to leave the comfort of your home. The SCAN Mobile Team, myself and the community all work together all work together to have your medical care brought to you!
If you are a resident of a long term care community and currently have Medicare Part A and Part B you may qualify for this plan.
