The Embrace plan and model of care are unique because we bring our Mobile Care team into the long term senior communities so our members don’t have to leave as often. You are able to enroll as long as you have Medicare parts A & B, live in a community such as an Assisted Living Community, Board and Care Home or Skilled Nursing Facility.
