The ISNP Embrace Plan is a Medicare Advantage plan specific for residents of long term care communities. It is designed to help keep you as healthy as possible without having to leave the comfort of your home.
If you are a resident of a long-term care community and currently have Medicare Part A and Part B you may qualify for this plan.
