The ISNP Embrace Plan is a Medicare Advantage plan specific for residents of long-term care communities. It is designed to be proactive and not reactive. Our whole goal is to increase communication with family and staff at the community in order to keep you as healthy and comfortable at home.
The Scan Mobile Team in partnership with the community staff work together to have your medical care brought to you! If you are a resident of a long-term care community and currently have Medicare Part A and Part B you may qualify to have your medical care brought to you at your community.
