If I don't select a Dental provider, how will a Dental provider be chosen for me?

If you do not select a dentist, one may be selected for you. You may however change dentists at any time. If you already have a dentist who is within the DeltaCare USA network of providers, and you know the Dental Office Name and the Dental Provider ID#, please enter the information in the fields above. If you do not know the information, or if you have not yet selected a DeltaCare USA dentist, you may visit www.deltadentalins.com and click “Find a Dentist” on the homepage. You may also call DeltaCare USA at 1-800-422-4234, 5 a.m.–6 p.m. Pacific Time, Monday–Friday or call SCAN Member Services Department at 1-800-559-3500, Hours are 8 a.m. to 8 p.m., 7 days a week from October 1 to March 31. From April 1 to September 30 hours are 8 a.m. to 8 p.m. Monday through Friday (messages received outside of our business hours will be returned within one business day). TTY users call 711.
