Access comprehensive care in the comfort of your home

As our patient, you receive access to a broad range of medical services all in the comfort of your own home. Our team also collaborates with everyone involved in your health and well-being in the community, along with your families, when desired. 

Prescribe, review, and refill medications

We regularly review medications prescribed to you to ensure they are safe and eliminate any that are unnecessary. On average, Medicare recipients are prescribed 18 medications, but we get it down to 6 medications per patient.

Chronic care management

We come up with a personalized care plan to manage your chronic health conditions.

Specialty care

If you need specialty care for your health concern, we will connect you with the right specialist and coordinate care between them and your SCAN Embrace (HMO I-SNP / HMO-POS I-SNP) team.

Behavioral health care

Our team of behavioral health clinicians provides a broad set of behavioral health services, including mental wellness check-ins, help managing behavioral health conditions like anxiety and depression, support navigating grief, and more.

Palliative care

We focus on enhancing the quality of life for individuals with serious illness. This includes psychosocial support and symptomatic management to keep you comfortable and keep conditions from progressing.

Advanced care planning

We’ll help you anticipate future health decisions and ensure you receive care that is aligned with your values, preferences, and goals.

Mobile labs and imaging

We'll bring lab testing and imaging studies to where you live. This includes mobile labs, X-ray, echocardiograms, ECGs, and ultrasound, among others.
