SCAN Case Management Programs

If you are (or are caring for) a SCAN member who has:
Two or more chronic health conditions, you may be eligible for our Care Management program. It is a challenge living with chronic conditions, such as diabetes and heart disease, so our Care Managers provide welcome education, coaching, and support. You will learn how to better manage your conditions, how to get the most out of the healthcare system, community resources, and the SCAN benefits available to you. Our Care Managers can also call on pharmacy, behavioral health, nutrition, and other health experts should you need extra help in these areas.
Congestive Heart Failure or Chronic Obstructive Pulmonary Disorder (COPD), you may be eligible for our Chronic Condition Management program. Our specially trained nurses will help you learn more about your heart or lung condition. They can talk to you about you treatment options, so you can better monitor and manage your health. They will work to improve communication with your doctor and enhance care coordination between your healthcare providers. The goal: better health and a better quality of life.
Recently been in the hospital (or are planning a hospital stay), SCAN’s Care Transitions team can help make the transition home a smooth one. Our specially trained Care Transition team can review hospital instructions, including any changes to medications, and make sure you know the symptoms to look out for. They can assist with obtaining required help or equipment in the home. Our care managers can also help with scheduling those important follow-up medical appointments and assist you in completing a Your Transition Home form to take to those appointments.
Program for Advanced Illness (PAI)
If you or your family member have a serious illness, such as late stage cancer, dementia, heart or lung disease, our Program for Advanced Illness may be able to help. Our specially trained nurses work with members & caregivers to manage pain and symptoms, coordinate care with your doctors, and help communicate your wishes for care.
Memory Program (MP)
If you or a family member has experienced a cognitive decline or have been diagnosed with Dementia, our Memory Program may be able to help. Our care managers work with members and caregivers to help manage these complex conditions, prevent caregiver burnout, coordinate care with medical providers, and provide referrals to community resources.
