What is Case Management?
Taking advantage of a SCAN Case Management program means that you can get extra help in taking care of your health. As part of one of these programs, you’ll be assigned a case manager who will work with your doctors and other healthcare providers to ensure you get the most out of your SCAN benefits.
Case management is most helpful if you have a chronic or complex healthcare issue. These programs are available to you, if you need them, at no additional cost. SCAN’s Case Management programs are described below. See which might work best for you.

Homebase Medical

Annual Wellness Visits
Homebase offers an in-home or virtual health assessment. This is a chance to get important information about your health, do preventive tests and screenings, discuss health concerns and learn about programs to manage your care.
Virtual visits are available in Arizona, California, Nevada, New Mexico and Texas. In-person visits are only available in Southern California.
Palliative Care
When you’re living with a serious illness, SCAN and Homebase’s Supportive Care for Advanced Illness (SCAI) program work to maintain your comfort. You’ll connect with an expert in home or by phone to ensure your quality of life is prioritized while managing your symptoms.
Available in Southern California.
Bone Density Screenings
If you’ve fallen or broken a bone in the last six months, you may need a bone density screening. Homebase can meet to do the screening in your home at your convenience.
Available in Southern California.
Care Transitions
If you’re recovering after a hospital stay, make the move back home easier with our support. Our care transitions program will connect you with a team member who makes sure your care needs are coordinated, so you recover comfortably and quickly.
Available in Southern California.
Members are referred to these programs by SCAN. Homebase will contact members to provide more information.
There is no charge for these services. The assessment(s), reports and other related materials or information are covered by SCAN.

Care Transitions
They can assist with obtaining required help or equipment in the home. Our case managers can also help with scheduling those important follow-up medical appointments. Members with ongoing case management needs will get referred to one of our other case management programs.
