Explanation of Benefits (EOB)

The Explanation of Benefits (EOB), or Monthly Summary, is summary of claims (bills) SCAN has processed. The EOB shows you the care you’ve received, what the plan has paid and how much you’ve paid out of pocket.

The EOB is not a bill; if you owe anything, your doctors and other healthcare providers will send you a bill. The EOB covers medical and hospital care only. You receive a separate summary for your Part D prescription drugs. You will not receive a summary if we have not processed any claims for that month.
