To review and/or download your EOC, in the boxes below, enter your language choice, the plan year, the SCAN plan you belong to, and the county in which you live.

NOTE: If you choose to have your EOC mailed to your home, all future EOCs will also be mailed to your home until you request otherwise by contacting Member Services.
