Call Member Services at 1-800-559-3500 for additional information. (TTY Users: 711). Hours are 8 a.m. to 8 p.m., seven days a week from October 1 to February 14.
From February 15 to September 30 hours are 8 a.m. to 8 p.m. Monday through Friday. Messages received on holidays and outside of our business hours will be returned within one business day.
Requesting an Exception to the SCAN Formulary
If you need to, you can ask us to cover a brand name drug or generic medication that isn’t on our list of covered drugs, or you can ask that we waive usage restrictions or limits for a certain drug. Generally, we’ll approve these “exception requests” when the drugs we do cover or the usage restrictions would’nt be as effective in treating your condition or would cause adverse medical effects.
When you request an exception, please include a statement from your prescriber or doctor that supports your request. Decisions are usually made within 72 hours of receiving that supporting statement. You can ask for an expedited, or faster, exception if you or your doctor believes that waiting up to 72 hours could be seriously harmful to your health. If your expedited request is granted, we must give you a decision within 24 hours of receiving the supporting statement from your doctor or prescriber.
