If you are receiving treatment (scheduled surgery, dialysis, chemotherapy, etc.) or using medical equipment issued by your doctor (C-PAP, Oxygen, Hospital Bed, etc.), please check “YES”. A SCAN Care Navigator will contact you near your enrollment date to assist with the transition of services.

Medication:
Please check the SCAN drug formulary list to see if your prescriptions require prior authorization. If so, please check “YES”. A SCAN Care Navigator will contact you near your enrollment date to assist you with your prescriptions.
