Why do I need to select a Dental provider?
You must choose a contracted Delta Dental provider that will provide and oversee your dental care. If you do not select a dentist, one may be selected for you. You may however change dentists at any time. If you already have a dentist who is within the DeltaCare USA network of providers, and you know the Dental Office Name and the Dental Provider ID#, please enter the information in the fields above
