Official Plan Materials
Medicare says we must send you these official plan materials. You can choose if you want to get them in the mail or online.
- Explanation of Benefits: This Monthly Summary of healthcare services is not a bill; it's for your information only. It lists the care you’ve received recently.
- Annual Notice of Change: This is sent every September. It outlines any changes to your SCAN plan for the coming year.
Personalized Notifications
- Health Check Record: This personalized record of your care is mailed once a year; the online version is interactive, updated monthly and available 24/7
- Personalized health information: Includes personal health information, such as specific medication names or other private info
- Portal notifications: We currently email you when you have a message in your SCAN member account. Uncheck the box if you don’t want these emails
