Communicating with a Care Team and Building Support — This session focuses on managing medical care including telemedicine, getting more from medical appointments, creating care plans and how to establish a support system.

Cualquier persona viviendo con diabetes sabe lo difícil que puede ser. Es por eso que SCAN Health Plan® está ofreciendo el taller Vivir bien con Diabetes y ¡está invitado a asistir! Este taller es un programa de 6 semanas en el cual usted y otros miembros de SCAN aprenderán los pasos que puede seguir para manejar mejor su condición de salud para que puedan hacer más de las cosas que les gusta hacer. Cada semana habrá una sesión en cual aprenderá los beneficios de actividad física, la alimentación nutritiva, la relajación y la importancia de comunicación con su grupo médico.
