The amount you must pay for health care or prescriptions before our plan begins to pay.

Department of Health and Human Services

The Centers for Medicare and Medicaid Services (CMS) is the agency within DHHS that administers the federal Medicare program.

Department of Health Care Services

The California State regulatory agency responsible for administering the Medi-Cal program.

Disenroll

When a member ends his or her membership in a Medicare Advantage plan. A member may disenroll from an MA plan during specified Election Periods. The member may also be involuntarily disenrolled by the MA plan only for certain reasons, including committing fraud or abuse, moving out of the plan's service area, or failing to pay plan premiums.

Drug Formulary

A continually updated list of the prescription medications covered by SCAN.

Dual Eligible

Person entitled to Medicare (Part A and/or Part B) and also eligible for Medicaid. Also referred to as "Medi-Medi."

Durable Medical Equipment (DME)

Certain medical equipment that is ordered by your doctor for medical reasons. Examples are walkers, wheelchairs, or hospital beds.

Durable Power of Attorney for Finances

An individual that a mentally competent member has designated, in writing, to make decisions and take necessary action related to the member’s funds or property when the member becomes legally incompetent. This authority remains in effect only as long as the individual is legally incompetent.

Durable Power of Attorney for Health Care

An individual that a mentally competent individual has designated, in writing, to make health care decisions and take action regarding the individual’s health care, including consent to medical procedures, based on the desires and wishes of the member. This authority becomes valid only when the individual becomes legally incompetent and remains in effect only as long as the individual is incompetent.
