SCAN Joins With the California Association of Long Term Care Medicine to Honor Providers for Innovation in Enhancing Care for Seniors

(LONG BEACH, Calif. – Jan. 21, 2016) — As part of its ongoing commitment to improve healthcare quality for older adults, SCAN Health Plan has joined with the California Association of Long Term Care Medicine to sponsor a Best Practices Implementation Award. The award will honor providers for innovation in processes focused on enhancing the treatment of patients with multiple chronic conditions and end-of-life care needs. 

Providers honored can be individuals or institutions that demonstrate the successful implementation of best practices along with a plan to ensure sustainability. These programs must be shown to yield positive care outcomes and have a proven track record that can be replicated throughout the long-term care community. The top three best practices will be honored at the 2016 CALTCM Annual Meeting on April 30.

“SCAN’s mission is built on making sure that seniors have unencumbered access to quality care that addresses their individual needs and allows them to remain healthy and independent,” said Dan Osterweil, MD, vice president and medical director at SCAN. “Our hope is that these best practices become standard in our industry so that all seniors can benefit.”

Judges have identified five finalists whose submissions cover the following themes: skilled nursing facility (SNF) collaboration as it pertains to transitions of care; decrease in the use of antipsychotics in the elderly; antibiotic stewardship specifically aimed at altering prescribing practices to eliminate inappropriate use of antibiotics; the use of life story boards to enhance the care of residents with dementia; and the reduction of 30-day SNF readmission. All will be presented at the April meeting, and the winning programs will be selected based on pre-determined criteria plus audience choice.

SCAN’s sponsorship of the Best Practices Implementation Award is the latest example of the health plan’s commitment to discover and disseminate innovation in senior healthcare. In November 2014 SCAN joined with the UCLA Multicampus Program in Geriatric Medicine and Gerontology to host a Best Practices Summit that called together 11 of California’s largest physician groups to share best practices and advance health plan-physician collaboration related to senior care. On the heels of that meeting, SCAN and UCLA honored three medical groups with a “Better Way to Care Award.”

In September 2015 SCAN recognized its top-ten performing medical groups who achieved an average of 4 stars or higher on SCAN’s internal quality metrics, which include a focus on preventive screenings, treatment of chronic conditions, and appropriate documentation to ensure that members get the care they need. And in November SCAN’s innovative Program for Advanced Illness, which helps members and their caregivers navigate the many health challenges associated with end-of-life care, was featured at the prestigious Healthcare Innovation Expo in Washington, D.C.
