Evidence of Coverage

The Evidence of Coverage (EOC) gives you details about your plan, including what’s covered, how your plan works, how much you can expect to pay and more.

To review and/or download your EOC, in the box below enter your language choice, the plan year, the SCAN plan you belong to and the county in which you live.

If you’d like a printed copy of your EOC mailed to your home, please click here and fill out the EOC request form.

NOTE: If you choose to have your 2019 EOC mailed to your home, all future EOCs will also be mailed to your home until you request otherwise by contacting Member Services.

Formulary

The SCAN Formulary is a list of prescription drugs covered by your plan.

To review or download your plan Formulary, please choose from the list below.

To search for a prescription drug in the Formulary, please use our Find a Formulary Drug search tool by clicking here.

If you’d like a printed copy of the Formulary mailed to your home, please click here and fill out the Formulary request form.

NOTE: If you choose to have your 2019 Formulary mailed to your home, all future Formularies will also be mailed to your home until you request otherwise by contacting Member Services.

The provider and pharmacy directory is a list of the doctors and pharmacy locations available to you through SCAN.
