SCAN Health Plan Launches Program for Managing Advanced Illness to Support Members and Caregivers

(LONG BEACH, Calif.) — SCAN Health Plan, one of the nation’s largest not-for-profit Medicare Advantage plans, has launched a case management program for members with advanced illness. The program is focused on enhancing quality of life; improving care coordination; and increasing member, family and caregiver satisfaction.
“SCAN has a long history of providing care management services that have a positive impact on the lives of our members and their families,” said Eve Gelb, senior vice president of healthcare services. “Coping with an advanced illness is a difficult experience for the member, family and caregiver. Our specially trained team is here to provide not only clinical support but the care and compassion necessary to manage a complex illness.”
Members enrolled in SCAN’s new case management program are supported by an interdisciplinary team of registered nurses, master’s-level social workers, gerontologists, a board-certified geriatrician, a registered dietician, a behavioral health specialist, and a clinical pharmacist. Diagnoses of members in the program may include congestive heart failure, chronic obstructive pulmonary disease, cancer, and end-stage dementia.
Through the program SCAN case managers coordinate care with the member’s physician(s) while providing emotional support to members and their families, including discussions about advance care planning and cultural and spiritual needs and preferences. Case managers also coach the member and caregiver on pain and symptom management, explain treatment options, and discuss what to expect if or when the disease progresses. In addition, case managers follow up with family members after the member’s death to offer bereavement resources or other services, if needed.
“Caregivers are an important focus of the program,” added Gelb. “Being a caregiver can be an overwhelming responsibility. Our case managers work to support and facilitate access to needed services for both the member and the caregiver.”
