If you are a staff person from one of SCAN's Physician Groups and would like to receive the latest copy of the 2008/2009 SCAN Benefits Grid, please email Jennifer Martin at mailto:jmartin@scanhealthplan.com and include the following information.

  • Title
  • Physician Group Name
  • Email Address
  • Phone Number
The sole purpose of the SCAN Benefits Grid is for our Physician Groups to update the copay information in their computer systems so they correctly reflect the current copayment amounts.
