COLLECTION AND USE OF PERSONAL INFORMATION
In conjunction with your registration for SCAN Health Plan’s (“SCAN”) Continuing Medical Education (“CME”) online program, you will be required to provide the following personally identifiable information:
- first and last name;
- home or other physical address;
- e-mail address;
- telephone number;
- the company for which you work;
- your occupation;
- your specialty;
- your license number.
The above-identified registration information is used to verify the validity of SCAN’s CME online program registrants and will not be supplied to any third parties or entities outside of SCAN. If, in the future, you wish to review or request changes to any of the personally identifiable information you provide in conjunction with your registration for SCAN’s CME online program, please do the following: sign in to your SCAN CME online account, click on the “Edit My Account” button to review and/or change your personal identifiable information.
SCAN will only use your e-mail address to notify you of upcoming CME activities and courses. Or, if you wish, you may opt out of receiving such e-mail notifications by: sending an e-mail to cme@scancme.com, with the words “OPT Out” in either the subject field or in the body of the e-mail.
SCAN maintains its own secure database that is not accessible to any third parties or other users which tracks the following information:
- Your username and password, via a cookie on your computer (if you have indicated you want this information remembered for you);
- Your user preferences and individual registration data;
- Your use of SCANCME.com, including your experience with the CME materials;
- Your certificate redemption records, in accordance with Accreditation Council for Continuing Medical Education (“ACCME”) guidelines.
CONFIDENTIALITY AND SECURITY
SCAN has implemented security measures in order to protect both personal and non-personal information from loss, misuse and unauthorized access, disclosure, alteration or destruction. SCAN’s employees are made aware of and are accountable for compliance with these procedures.
CHANGES TO THIS PRIVACY POLICY
In the event that SCAN makes any material changes to this privacy policy or its CME online website, SCAN will notify you, the user, by posting a clear and conspicuous notice of these changes on the CME website and in this privacy policy.
EFFECTIVE DATE
In accordance with California Code - Chapter 22: Internet Privacy Requirements Sections 22575 – 22579, this Internet Privacy Policy is effective on May 14, 2010.
