 |  Q: |  Why is SCAN a not-for-profit organization? |

Almost 30 years ago, a group of seniors grew frustrated by how difficult it was to get quality medical and in-home health care services. They decided to form a not-for-profit organization so other seniors could get those benefits conveniently and affordably. Together with a group of experts in medicine, gerontology, psychology and social services, they formed the grassroots organization Senior Care Action Network. Today, SCAN Health Plan provides seniors with the widest array of health care and in-home services available – with the goal of helping them maintain their health and independence. Back to questions

SCAN is one of the few Medicare health plans in the country entitled to offer personal care and in-home benefits that can help keep you independent. So, you get all the advantages of Medicare, plus access to much more. In addition to quality health care services, if you qualify and are approved to receive these additional benefits, you can also get Independent Living Power® – the full suite of SCAN in-home and personal care services.  |  Q: |  Who qualifies for SCAN’s personal care and in-home services, called Independent Living Power®? |

Independent Living Power® is available to SCAN members residing
in Los Angeles, Orange, Riverside and San Bernardino counties
who, after an assessment by a SCAN Personal Care Planner®, are
determined to need help to stay at home safely and independently.
Your SCAN Personal Care Planner helps identify your needs and,
when you qualify and are approved to receive additional benefits,
organizes the services you receive into a personal care plan to
ensure your needs are met.

 |  Q: |  If I switch to SCAN, can I keep my current doctor? |
 | A: |
SCAN has relationships with many quality doctors in each
community where it provides coverage, including leading medical
groups and respected hospitals. Members may choose from these
doctors and hospitals, and many seniors will find their current
doctor, specialists or preferred hospital already accepts SCAN.
As a SCAN member, you must use SCAN providers for routine care.

 |  Q: |  How do I enroll in SCAN Health Plan? |

 |  Q: |  What are the requirements to join SCAN? |
 | A: |
- You must reside within a SCAN approved service area - which includes most of Kern, Los Angeles, Orange, Riverside, San Bernardino, San Diego and Ventura counties.
- You must have Medicare Parts A and B
- You do not have End Stage Renal Disease [ESRD]
- ESRD is permanent kidney failure and requires regular kidney dialysis or a transplant to stay alive. If you have ESRD, you cannot enroll in this plan unless you were previously enrolled in a Medicare Advantage Plan that left the Medicare program or stopped providing coverage in their area on or after December 31, 1998.
