To assist you in making an informed decision about SCAN, below you will find links to our benefit materials.
SCAN benefits, formulary, pharmacy network, premium, co-payments and/or co-insurance may change on January 1, from year to year.
SCAN offers verbal interpreter services in your language. These materials may be available in other languages. SCAN also offers basic enrollee information in other formats to accommodate beneficiaries with disabilities. Please call SCAN for details, 1-888-540-7226, 7:00 a.m. – 8:00 p.m., seven days a week. TTY users call 1-800-367-8939. This information is available for free in other languages. Please contact our customer service number at 1-888-540-7226 for additional information.

SCAN offers a network of primary care doctors, specialists and hospitals. You must receive all routine care from in-network plan providers; except for emergent or urgent care situations or for out-of-area renal dialysis. If you obtain routine care from out-of-network providers neither Medicare nor SCAN will be responsible for the costs.

The Centers for Medicare & Medicaid Services values your feedback and will use it to continue to improve the quality of the Medicare program.
