To join SCAN Health Plan Arizona, you must meet the following requirements:

Maricopa County resident
Medicare (Part A&B) eligible


	Join SCAN Online
	Click the button below to go to the SCAN online enrollment page. There, you can enroll in SCAN by completing and submitting the online form. A SCAN representative will contact you within 3 business days to confirm receipt of your online enrollment form.
