SCAN Health Plan is a Medicare Advantage Organization with a Medicare contract.
Individuals must have both Part A and Part B to enroll. You must continue to pay your Medicare Part B premium. Members may enroll in the plan only during specific times of the year. [1]There are limited exceptions to the restrictions to the time of year when a beneficiary may enroll in our plan. Call SCAN to learn more about the plans offered by SCAN, and whether you qualify for a Special Election Period (SEP).
- SCAN Health Plan is Available in the Following California Counties: Los Angeles, Orange, Riverside, San Bernardino, San Diego, Ventura, Kern, San Francisco, Santa Clara, Contra Costa and San Joaquin Counties.
- SCAN Health Plan is available in the following Arizona counties: Maricopa & Pima.
[1]SCAN offers the following Coordinated Care, Special Needs Plans: SCAN Connections (HMO SNP), SCAN Connections at Home (HMO SNP), SCAN Healthy at Home (HMO SNP), Heart First (HMO SNP), SCAN Balance (HMO SNP), SCAN Plus (HMO SNP) & [2]VillageHealth (HMO-POS SNP) (VillageHealth is an open access plan designed for those diagnosed with ESRD. VillageHealth members may see any provider who accepts Medicare with no change to your out-of-pocket share of cost. VillageHealth members must use network pharmacies to access prescription drug benefits, except under non-routine circumstances).
Benefits, formulary, provider network, pharmacy network, premium, co-payments and/or co-insurance may change on January 1 of each year. [2]You must receive all routine care from plan providers. You may be able to get Extra Help to pay for your prescription drug premiums and costs. To see if you qualify for extra help, call: 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048, 24 hours a day/7 days a week; The Social Security Office at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday.
